Comprehensive Health and Decision Information System
The Pediatric Care Center is excited to announce we have now partnered with CHADIS, a service that provides a central platform for developmental, behavioral and mental health resources all in one convenient and secure online format. It allows you to complete questionnaires related to your upcoming visit whenever it is most convenient for you.
CHADIS was established in 2001 by Drs. Barbara Howard and Raymond Sturner, both physicians and leaders in Developmental-Behavioral Pediatrics at Johns Hopkins in conjunction with The Center for Promotion of Child Development through Primary Care. The Center’s mission is to conduct research and further develop computerized methods of assisting primary care clinicians in the detection, treatment, and prevention of mental, behavioral and developmental problems, as well as promoting positive development in all children, adolescents and young adults. The Center focuses on developing capabilities to enhance routine check-up visits, including those with special health risk conditions.
When you use CHADIS, you provide your child’s doctor with information in advance of your appointment so you can save yourself time in the office and help your provider be best prepared to address your needs during the visit.
Through email, you will be invited to:

Register for CHADIS up to one week in advance of your appointment

Follow the instructions to register

Once you have completed your one-time registration, age-appropriate questionnaires will be assigned for the upcoming visit

We will continue to contact you one week before future visits to enable access to appropriate questionnaires for those visits

Questionnaires will take about 10-20 minutes to complete (See How)
